Patient care is considered one of the most important and essential medical tasks performed by medical professionals. Patients often suffer from co-morbidities, which requires the support of collaboration and partnerships with other relevant organizations and professionals who can jointly address patients’ various health and psychological needs. , resulting in effective and better health outcomes. Patient care team

It is clear from Liam’s case study that he suffers from both physical and mental health issues. Physically, I have had JIA (juvenile idiopathic arthritis) or polyarthritis since I was 8 years old, and I have had asthma since I was 10 years old. Both physical ailments worsened and he was no longer able to participate in school activities. This therefore had a huge impact on Liam’s mental health and wellbeing. It is important to note that both physical and mental illnesses are intertwined and influence each other. Therefore, Liam’s mother reports that Liam suffers from depression and social isolation, which causes him to withdraw from all social activities, and he actively takes care of his health. The multidisciplinary health team is made up of nurses, dietitians, staff, physicians, psychologists, social nurses, and community nurses who work together to provide high-quality, coordinated care to patients and their It brings family satisfaction (Soukup et al ., 2018). . Liam also has various types of health issues. Therefore, different health professionals should consider his different health needs as it leads to better decision making and faster results (Soukup et al., 2018).

Therefore, the first members of the care team include a public health physician and a physician who manage the complexities of Liam’s physical health, prescribe medications, and assess his progress (Nilsson et al., 2020) . The second member of the medical team consists of a nurse who administers Liam’s medications, provides emotional support to Liam and his family, and advises concerned doctors and family members of any improvement or deterioration in Leanne’s condition. (Nilsson et al., 2020). In addition, nurses will educate Liam and his family on how to provide independent care at home without hospitalization. The nurses will also be responsible for teaching Liam and his parents strategies to manage his chronic illness at home and elsewhere, which will help improve Liam’s self-esteem (Pfob et al., 2021).

In addition, the medical team will also include a pediatrician who will not only provide medical care to Liam, but also inform Liam and his family of some of the preventive health services that will contribute to better health and treatment outcomes for Liam. (Pfob et al., 2021). The medical team will also include a physical therapist who will teach Liam and his parents appropriate exercises to control the pain and stiffness in Liam’s body, which suffers from JIA. Understanding and practicing exercise will help Liam manage his pain, which will also encourage him to participate in different activities.

Barriers To Care

Another essential member who needs to be included in the care team is a psychotherapist or a counsellor who will be responsible for treating Liam off his mental health condition of depression and social isolation by offering him relevant therapies that will boost his confidence, make him overcome his negative feelings and replace them with positive thoughts and ideas (Wood et al., 2018). The counsellor or therapist will also be offering therapy to Liam’s parents since their mental condition too might be affected with continuous deterioration of their son’s health which will thereby help them to effectively participate in their son’s management of mental and physical health conditions (Wood et al., 2018).

A care coordinator will also be included in the multidisciplinary team who will be monitoring the effectiveness of physical and psychological interventions for the next 6 months and ensure that health outcomes are positive and satisfactory for both Liam and his parents (Proctor & Vu 2019).

Social care providers will also be included in the care team who will be making Liam’s family aware of services and programs that will help in lessening their financial burden and will also make them aware of their rights as parents and the type of treatment and services that they can expect from the healthcare organization and the healthcare team in particular (Sinclair, 2019).

Hence, the two chronic illnesses of JIA and asthma are the primary health concerns that need to be addressed by the healthcare since it is due to this reason, that Liam’s mental health condition is being affected. On the other hand, Liam’s psychological state should be considered as a secondary element of care, which ultimately contributes to supporting and encouraging Liam and his parents in the treatment process.

This section describes some of the obstacles that Liam and his parents may face during care treatment and its implementation in the medical field. One of the first barriers to achieving care goals is the multidisciplinary team’s inability to understand Liam’s care and treatment needs (Murphy et al., 2019). In other words, the lack of skills and knowledge of the multidisciplinary team resulted in an inability to effectively understand Liam’s mental and physical health issues, resulting in a lack of understanding of the medication process and support for those affected. (Murphy et al., 2019).

A lack of skills and expertise in the care team could delay the treatment process and result in Liam and his parents receiving inappropriate care and medical services (Doukanari et al., 2021). Disagreements and lack of coordination and respect between members of the multidisciplinary team are another significant obstacle that greatly impacts the achievement of the care goals set for Liam (Doukanari et al., 2021). For example, different cultural or medical beliefs among team members can influence the decision-making process and lead to delays in treatment.

Lack of resources and funding is another barrier that impacts care goals. Without funding and resources, it is not possible to support health activities and programs that impact Liam’s treatment process (Aldawood et al., 2020). On the other hand, if Liam’s parents have financial problems, they will not be able to access the services of a multidisciplinary team, nor will they be able to access medical resources that could help treat Liam (Aldawood et al. ., 2020).

Therefore, lack of funding and resources will ultimately aggravate the physical and mental health condition of Liam. Lack of staff within the healthcare organization and lack of communication abilities will again act as a barrier in meeting care goals of Liam since different specialists and experts will not be able to meet different needs of Liam and his family that will lead to delay in care and will also not allow the available health practitioners to deliver quality care since they might not have awareness regarding how to treat different health conditions (Rosen et al., 2018).

Lack of adherence and respect for the cultural and religious beliefs of Liam and his family is another significant barrier that might affect the care process. In other words, all the members of the multidisciplinary team should ensure that the treatment they will offer Liam and his family will be clearly aligned to their values and beliefs that will motivate them to respond to treatments which will thereby help in reducing the impact of mental and physical health condition on Liam (Rosen et al., 2018).

Some of the care coordination skills, that I, as a registered nurse will demonstrate to effectively manage the shared care plan of Liam is to practice critical thinking skills that will help me to effectively identify barriers to care which will help me to meet the care goals that have been identified after assessing the health of Liam. If critical thinking skills are not utilized, the diverse health needs of patients will not be recognized and will inevitably impact the care process (Hewes et al., 2021).

Time management is another care coordination skill that I need to adhere to. This helps make quick and appropriate decisions and find appropriate interventions to prevent delays in care, thereby helping to achieve care goals in a timely manner and also overcoming barriers to care (Hewes et al., 2021).

There are several technologies that can help meet Liam’s medical needs. For example, the medical team has access to telemedicine, allowing Liam and her family to access support and guidance from each member of the multidisciplinary facility via their cell phones and laptops (Weinstein et al., 2019). Wearables are one of the latest technologies that medical professionals can use to monitor a patient’s condition throughout the day from the comfort of their own home, while also assessing the progress and effectiveness of the care plan and identifying necessary We also provide help and advice depending on your needs (Weinstein et al, 2018).
